Ο Martin είναι ένα διαταραγμένο διανοητικά, εσωστρεφές άτομο που ζει με τη μητέρα του σε ένα ζοφερό σπίτι. Εργάζεται σαν νυκτερινός φύλακας ασφαλείας σ ένα εξίσου ζοφερό και αποκαρδιωτικό υπόγειο πάρκινγκ. Για να ξεφύγει από την θλιβερή ύπαρξή του, ο Martin χάνεται στον φανταστικό κόσμο της cult ταινίας τρόμου "The Human Centipede (First Sequence) (2009)", και κάνει φετίχ τις σχολαστικά χειρουργικές ικανότητες του χαρισματικού Δρ. Heiter, του οποίου η γνώση για το γαστρεντερικό σύστημα του ανθρώπου, εμπνέει τον Martin να επιχειρήσει το αδιανόητο.
